What is included?
The template includes the task, assessment conditions, observable criterion, accepted evidence, observer, observation date, outcome and follow-up. Open the spreadsheet in your spreadsheet application and save a working copy before entering your own information.
The sample describes checking a customer request and escalating an exception. It illustrates how to write a criterion; it is not an approved assessment standard. Replace it with requirements agreed by your organisation.
Define the criteria before the observation
Describe what the person should do and the conditions under which they should do it. For example: identify the request, follow the approved procedure and escalate an exception to the named role. Agree what evidence would demonstrate each step.
Use one row per criterion. A criterion that combines several unrelated tasks can hide a gap behind an overall pass.
Record evidence and a clear outcome
Record what was observed rather than only a score. Suggested outcomes are not observed, meets criterion and further practice needed. Record the reviewer and date, and include an evidence reference where useful.
An overall competency decision belongs to an authorised reviewer. The template does not calculate or certify competence. Keep personal and sensitive information within your organisation’s approved storage process.
Follow up on the gap
If further practice is needed, record the action, owner and review date. Preserve earlier observations so another reviewer can understand what changed.
When version control and reviewer follow-up become difficult, evaluate a connected workflow using one task and a small group before transferring a larger record set.
